Facility failed to maintain kitchen in a sanitary manner and ensure food was prepared and served according to Food Sanitation Rules. Multiple areas of food spills, dirt, grease, and needed repairs were observed. Staff did not sanitize thermometers or change gloves appropriately. Food contact surfaces were cluttered and unclean.

Multiple deficiencies including failure to treat residents with dignity during meal delivery, service plans not reflective of residents' current needs, failure to conduct unannounced fire drills per Oregon Fire Code, and heating elements exceeding safe temperatures. None of the deficiencies were corrected as of last revisit.

Complaint investigation found failure to provide sufficient qualified awake direct care staff to meet residents' needs and failure to fully implement and update acuity-based staffing tool. Deficiencies were not corrected as of last visit.

Facility failed to ensure kitchen practices and protocols met Food Sanitation Rules. Multiple areas of food debris, grease buildup, and lack of beard restraints were observed. Deficiencies remained uncorrected as of last revisit.

Facility failed to immediately notify Department Central Office and local public health authority of epidemic disease outbreak. Outbreak involved multiple residents and staff with gastrointestinal symptoms. Findings were acknowledged but not corrected as of last visit.

Facility was in substantial compliance with Resident Services - Meals and Food Sanitation Rules. No deficiencies identified.

Multiple deficiencies including failure to investigate and report injuries of unknown cause, incomplete move-in evaluations, service plans not reflective of residents' needs, inadequate fire and life safety drills and training, and failure to ensure staff training compliance. Some deficiencies were corrected on revisit, others remained uncorrected.

Complaint investigation found failure to comply with proper infection control including staff not wearing face shields or masks appropriately. Deficiencies were not corrected as of last visit.
