Visit Reason
A fire life safety and sanitation licensure survey was conducted to assess compliance with safety regulations.
Findings
Two deficiencies were identified related to prohibited electrical applications and inadequate fire drill documentation.

.405.02.a Prohibited applications: during facility tour, a relocatable power tap was used to supply power to an oxygen concentrator, which is a prohibited use.
.410 Requirements for emergency actions and fire drills: the facility failed to conduct the required two fire/evacuation drills during the night when residents are sleeping, and documentation was incomplete.

A health care initial licensure survey was conducted to evaluate compliance with Idaho RALF regulations.
Findings
Three deficiencies were found related to incomplete background checks, inadequate documentation of residents' negotiated service agreements, and incomplete documentation of quarterly resident assessments.

.009.06.c Use of previous criminal history and background check: three of seven employees did not have an Idaho State Police background check completed, confirmed by the facility administrator.
.320.01 Use of NSA: residents' negotiated service agreements did not clearly reflect specific needs or describe services provided, including money management, shopping, laundry, housekeeping, night needs, and emergency evacuation.
.330.04.c.vii Resident care records: the facility RN assessed seven residents for quarterly assessment but did not document all areas assessed due to limitations of the computer system.
