On or about October 14, 2019, surveyors conducted an investigation into three complaints regarding the Maybelle Carter Senior Adult Home.
Complaint Details
The investigation was complaint-related based on three complaints. The survey confirmed serious violations that endangered resident health and safety.
Findings
Surveyors observed serious violations of state laws and regulations resulting in endangerment to the health, safety, and welfare of residents. Issues included gnats/small flies in storage and dish machine areas, food spillage, accumulated debris, and broken floor tiles in the kitchen area.

Rule 1200-08-25-.07(7)(c)(5) requires an assisted-care living facility to maintain a clean and sanitary kitchen. The facility had gnats/small flies in the dry goods storage room and dish machine area, food spillage under the food preparation area, accumulated debris on the gas range, broken baseboard floor tiles near the dish machine and steam table, and debris on the dish machine surface, wall, and floor.
