The Department conducted a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.

Findings

WAC 388-78A-2474 Training and home care aide certification requirements. The facility failed to ensure 1 of 6 sampled staff met training requirements for long-term care workers, placing residents at risk of harm and care from untrained staff.

The inspection was conducted as a complaint investigation triggered by allegations of resident neglect and inappropriate discharge at the assisted living facility.

Complaint Details
The complaint investigation involved two allegations: 1) Resident neglect for locking a resident outside during a behavioral crisis, which was substantiated with a citation written. 2) Inappropriate discharge of a resident, which was not substantiated due to insufficient evidence. The investigation included interviews, record reviews, and observations.
Findings
The investigation found one instance of failed provider practice where a resident was locked outside during a behavioral crisis without adequate monitoring, placing the resident at risk. Another allegation regarding inappropriate discharge was not substantiated. The facility was found not in compliance with licensing laws due to the monitoring failure.

The inspection was conducted as an unannounced complaint investigation regarding the facility's policy of calling EMS or fire department to lift residents from the floor instead of facility staff lifting them.

Complaint Details
Complaint #109819 alleged that facility staff called EMS/9-11 only to lift an uninjured resident from the floor. The investigation confirmed the allegation and cited the facility for failing to have appropriate policies and procedures to safely lift residents without outside assistance.
Findings
The investigation found that the facility failed to ensure residents could be safely lifted from the floor by staff without outside assistance, placing one resident at risk. The resident was not injured, but the facility policy requires calling EMS or fire department for lifting. A citation was issued for noncompliance with WAC 388-78A-2600 (1b).

The Department completed a full inspection of the Assisted Living Facility to assess compliance with Assisted Living Facility requirements.

Findings
The facility was found not to meet Assisted Living Facility requirements due to loose pavers on the common patio outside the dining area. The facility administrator and maintenance director implemented a safety plan and planned to begin repairs within a week.
