The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00176830 conducted on December 5, 2023:
Findings
Two deficiencies were found related to medication administration and storage of toxic materials. The facility failed to administer medication according to the physician's order and did not store toxic materials in a locked area inaccessible to residents.

A manager failed to ensure a medication was administered in compliance with a medication order for one resident, resulting in a discrepancy between the medication order and the medication administration record.
A manager failed to ensure toxic materials were stored in a locked area inaccessible to residents, as several cleaning products were found unlocked in a kitchen cabinet.
