Visit Reason
On-site compliance (annual) inspection of an Assisted Living Home conducted on October 20, 2025.

Findings
The inspection found nine deficiencies related to tuberculosis infection control, opioid medication documentation, residency agreements, service plan documentation, medical record protection, medication administration compliance, medication storage, disaster planning, and environmental cleanliness. Plans of correction were provided for all deficiencies.

Deficiencies (9)
R9-10-113 — The facility failed to annually assess the risk of exposure to infectious tuberculosis, posing a TB exposure risk to residents. No facility risk assessment was documented or available during the inspection.
R9-10-120 — The manager failed to ensure documentation in a resident's medical record of the need for opioid administration and the effect of the opioid administered. Documentation was missing for one of nine residents sampled.
R9-10-807 — The manager failed to ensure residents provided evidence of freedom from infectious tuberculosis before or within seven days after occupancy for two residents. No documentation was found in their medical records.
R9-10-808 — The manager failed to ensure caregivers documented services provided in residents' medical records for two residents. Documentation of required services such as night checks, showers, and dressing was missing.
R9-10-811 — The manager failed to protect residents' medical records from loss, damage, or unauthorized use. Medical records with private information were observed left out on a table.
R9-10-817 — The manager failed to ensure medications were administered in compliance with medication orders. One resident received acetaminophen less frequently than ordered, and another resident had no documentation or administration of prescribed Lyrica.
R9-10-817 — The manager failed to ensure medication was stored in a locked, separate area. Unlocked medications were found in a resident's bedroom, posing a risk to residents.
R9-10-819 — The manager failed to ensure the disaster plan included a plan to make each resident’s medication available during a disaster. The plan lacked this critical component, risking resident health and safety.
R9-10-820 — The manager failed to ensure the premises were cleaned and disinfected properly. Spoiled and moldy food was observed in the kitchen refrigerator, posing a risk to resident health.
