Annual Licensure Survey combined with Facility Reported Incident to 2/7/26, IL200893.
Findings
The establishment was cited for multiple deficiencies including failure to orient residents to emergency evacuation, incomplete employee verifications and health evaluations, failure to revise resident service plans after falls, lack of tuberculosis testing for residents and staff, medication administration errors including a significant omission of a cancer medication for a resident, and failure to report the medication error timely. The facility does not perform emergency evacuation orientation or obtain resident signatures for it.
