Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on April 11, 2024.

Emergency call response: Documentation of hourly safety checks for 59 Special Care Residence Residents had multiple instances where staff failed to document the hourly safety checks during the hours of 7:00 p.m. and 7:00 a.m.
Quality assurance program: The Executive Director did not document an investigation into the allegation of misuse of Resident finances, missing required forms and failing to follow the Resident Abuse Policy, including timely investigation and notification.
Incident reporting: The Residence filed 17 incident reports greater than 24 hours after the occurrence of the incident or accident (repeat finding).
Resident progress notes: Inconsistent documentation of significant occurrences in the Progress Notes for two Residents, including missing documentation of an allegation of Resident financial abuse.
