Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on April 25, 2024.

Findings
The state cited 6 findings, including 4 repeat findings, matching the state median of 6. The action taken was a plan of correction required.

Emergency call response: In the months of November 2022, June 2023 and March 2024 there were 758 e-call response times over the ten (10) minutes required by Residence policy (repeat finding).
Memory care unit: Documentation to support that the Residence conducted SCR operations reviews was missing for the second half of 2022, second half of 2023, and first half of 2024 calendar years reviewed. Documentation of a physical/occupational therapist assessment every six months was missing for six Residents utilizing bed rails/U-bars for the calendar years reviewed.
Resident screening and assessment: Missing Bedrail assessments.
Quality assurance program: Documentation of target dates for follow up action and clear outcomes was missing for 2022 and 2023 calendar years. Medication Quality Reviews for the last two quarters of 2023 and all quarters of 2023 had no findings, but the Residence reported 17 medication errors since the last compliance review. Observed Personal Care staff providing [redacted] outside the scope of SAMM and a compromised medication blister stored in a Resident’s medication storage unit (repeat finding).
Emergency drills · Incident reporting: Documentation of elopement drill participant signatures was missing for the second shift for 2023. The Residence filed 69 incident reports greater than 24 hours after the incident or accidents (repeat finding).
Staff training: Eight records lacked documentation to support that Introductory Visits were consistently conducted with all applicable staff prior to or within 48 hours after the provision of Personal Care services, at move-in or change in condition (repeat finding).
