The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.

Findings
The follow-up inspection found no deficiencies and confirmed that all previously cited deficiencies were corrected.

Deficiencies (2)
WAC 388-78A-3090 Maintenance and housekeeping. The facility failed to provide a safe, sanitary, and well-maintained environment in three locations including first floor, second floor, and outside grounds, placing all residents at risk. Observations included black marks on doors, dried liquids on walls, missing drawer knobs, cobwebs, broken ceiling tiles, dust buildup, disorganized storage, and unsafe outdoor conditions.
WAC 388-78A-2466 Background checks. The facility failed to ensure 2 of 6 staff had valid Washington State background checks renewed every two years, placing resident safety at risk. Staff E and F had background checks renewed late by 16 and 110 days respectively.

Visit Reason
The inspection on 04/02/2025 was conducted as a follow-up to verify that all violations noted during previous related inspections had been corrected.

Complaint Details
The complaint investigation related to a fire incident on 03/17/2025 at Brookdale Everett involved a resident who was injured. The cause of the fire was unknown and under investigation. No sprinkler activation occurred, no other residents were evacuated, and one resident was injured. The complaint number was 171133.

All violations from prior inspections have been corrected, resulting in an Approved status for this inspection.

Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the residential care facility Brookdale Everett.

Findings
The inspection identified multiple fire safety violations including combustible materials stored improperly, penetrations in fire-resistance-rated construction, and issues with smoke detector placement. Some violations were corrected on site, but the overall approval status was Disapproved.

Deficiencies (8)
IFC 315.2.3 (2021) - Combustible material shall not be stored in boiler rooms, mechanical rooms, electrical equipment rooms or in fire command centers. There was combustible storage within the mechanical furnace room near 227.
IFC 703.1 (2021) - Materials and firestop systems used to protect membrane and through penetrations in fire-resistance-rated construction shall be maintained with no openings visible through or into the cavity of the construction. There was a through penetration of the ceiling assembly next to the smoke detector near 110.
IFC 903.5 (2021) - Sprinkler systems shall be tested and maintained in accordance with Section 901. The walk-in type cooler and freezer has ordinary temperature heads installed.
IFC 907.8 (2021) - The maintenance and testing schedules and procedures for fire alarm and fire detection systems shall be maintained. The smoke detector head located near 217 has not been installed within 36 inches of an air supply diffuser or return air opening.

Electrical equipment rooms, fire command centers, fire pump rooms, generator rooms, and public restrooms with area greater than 300 square feet shall be illuminated.
IFC 1203.4 (2021) - Emergency and standby power systems shall be maintained to supply service within the time specified. The system was maintained in accordance with NFPA 110 and NFPA 111.
