Original Licensing
Date: Apr 25, 2024

Visit Reason
A health care initial licensure survey was conducted to evaluate compliance with licensing requirements.

Findings
Four deficiencies were found related to notification to the licensing agency, medication distribution system, medication destruction procedures, and food and nutritional care services.

Deficiencies (4)
.215.08.f. Notification to Licensing Agency within One Business Day: the facility did not notify Licensing and Certification within one business day when residents fell or sustained injuries, due to misunderstanding of reporting requirements.
.310.01.c. Medication Distribution System: the facility did not maintain medication refrigerator temperatures between 38 and 45 degrees F, with 68 out-of-range occurrences between February and April 2024, and staff were unclear about monitoring responsibilities and policies.
.310.02. Discontinued and Expired Prescriptions: the facility did not consistently have a witness for medication destruction, and destruction logs lacked documentation; the lead CNA was unaware a witness was required and the facility used kitty litter to destroy medications.
.450. Requirements for Food and Nutritional Care Services: the facility did not have a Certified Food Protection Manager at the time of survey; the former manager's certification had expired and the kitchen manager in training was obtaining certification.
