Visit Reason
The inspection was conducted as an End of Provisional Survey to assess compliance with licensing and regulatory requirements for a Level II Residential Care Facility.
Findings
The facility was found not in compliance with licensing regulations due to failure to obtain comprehensive background checks for three employees and lack of a current written consent to release information in one resident's record.
Deficiencies (2)
Facility did not utilize the Maine Background Check Center to obtain comprehensive background check reports for Employee #1, Employee #2, and Employee #3.
Resident #1's record did not contain a current written consent to release information to the resident's family member.
