The following deficiencies were found during the on-site compliance inspection and investigation of complaint 00135537 conducted on July 17, 2025.
Findings
This inspection found seven deficiencies related to medical record protection, medication orders, vaccination documentation, resident retention requirements, exit alarm functionality, medication storage security, and environmental safety hazards.
Deficiencies (7)
R9-10-811 — The manager failed to ensure residents' medical records were protected from loss, damage, or unauthorized use, as records were observed in an open area accessible to unauthorized persons.
R9-10-811 — The manager failed to ensure a resident's medical record contained a medication order signed by a medical practitioner for each administered medication, preventing verification of medication administration.
R9-10-811 — The manager failed to ensure documentation of notification to a resident about the availability of influenza and pneumonia vaccinations was present in the medical record.
R9-10-814 — The manager retained a resident confined to a bed or chair without current documentation from the primary care provider confirming the resident's needs could be met by the facility.
R9-10-815 — The manager failed to ensure a means of exiting the facility for residents without keys or special knowledge was monitored or alerted staff, as the back door alarm was observed turned off.
R9-10-817 — The manager failed to ensure medication was stored in a locked area used only for medication storage; insulin and medication carts were observed unlocked and accessible.
R9-10-820 — The manager failed to ensure the premises were free from conditions that could cause physical injury, as a broken glass shard was found accessible on a table near the backyard door.
