The following deficiencies were found during the on-site investigation of complaints 00155816 and 00155822 conducted on January 12, 2026.
Findings
The inspection found seven deficiencies related to personnel documentation, tuberculosis screening, personnel records, service documentation, exit door alarms, medication administration documentation, and medication storage security.

R9-10-806 — The manager failed to ensure accurate documentation of caregivers' schedules, including days and hours worked, resulting in discrepancies such as terminated staff still listed as working and unupdated schedule changes.
R9-10-806 — The manager failed to ensure a caregiver provided evidence of freedom from infectious tuberculosis on or before the date services began, lacking required documentation for one of three personnel records reviewed.
