Visit Reason
A health care licensure and follow-up survey was conducted to assess compliance with state regulations.
Findings
Three non-core deficiencies were found related to plumbing water temperatures, medication refrigerator temperatures, and food and nutritional care standards.
Deficiencies (3)
.250.09 Plumbing: the facility did not maintain water temperatures between 105 degrees F and 120 degrees F; temperatures ranged from 133 degrees F to 99 degrees F in building #1 from 4/7/25 to 4/8/25, and the administrator noted ongoing issues with the water heater.
.310.01.c Medication Distribution System: the facility did not maintain medication refrigerator temperatures between 38 and 45 degrees F; documented lows included 35 degrees in all three buildings, with temperatures below 38 degrees 51, 33, and 29 times respectively, and the administrator was unaware that food and medication temperatures needed to be maintained separately.
.450 Requirements for Food and Nutritional Care Services: the facility failed to meet Idaho Food Code standards as the kitchen inspection failed on 4/8/25, requiring mandatory reinspection within 10 days; staff require additional training according to the administrator.

A fire life safety and sanitation licensure survey was conducted to assess compliance with fire and life safety standards.
Findings
Six deficiencies were found related to fire and life safety records, electrical installations, and prohibited applications involving power equipment.

Fire and Life Safety Records: facility failed to maintain fire and life safety records and could not provide documentation for the five-year internal obstruction investigation of the fire suppression system in buildings #1 and #2, including replacement or recalibration of water pressure gauges and check valve inspection at a five-year interval.
.405.01.a. Electrical Installations and Equipment: during facility tour of building #3, Room #5 was observed using a multiplug adapter to power a miniature refrigerator; the use of multiplug adapters is prohibited.
.405.02.a. Prohibited Applications: during facility tour of building #3, Room #4 was observed using a relocatable power tap (RPT) to supply power to an oxygen concentrator; the use of a RPT to power medical equipment is prohibited.

A health care initial licensure survey was conducted to evaluate compliance with state regulations.
Findings
Six deficiencies were identified related to medication administration, documentation, service agreement updates, infection control, and staff training.
Deficiencies (5)
.305.02.b. Current Medication Orders and Treatment Orders: the facility nurse did not ensure residents received medications and treatments as ordered, including incorrect dosing frequencies and failure to crush medications as ordered.
.310.02.f. Discontinued and Expired Prescriptions: the medication destruction log did not document two signatures for each medication destroyed, missing the second signature from 2/7/24 to 3/21/24.
.320.08. Periodic Review: residents' Negotiated Service Agreements were not consistently updated to reflect significant health status changes, including feeding assistance and pressure injury care.
.335.02. Standard Precautions: infection control procedures were not followed, including failure to sanitize hands between medication passes, improper handling of medication spoons, lack of glove use, and absence of paper towels in a shared bathroom.
.620. REQUIREMENTS FOR TRAINING OF FACILITY PERSONNEL: staff were insufficiently trained on behavior management plans, pureed diet consistency, safe resident transfers, and medication destruction policies.
