Complaint Details
An on-site investigation of complaint AZ00218879 was conducted on November 18, 2024, and the following deficiencies were cited.
Findings
The inspection found seven deficiencies related to staff training, supervision, documentation, and tuberculosis screening. The facility failed to ensure proper CPR training, valid caregiver certification, adequate supervision of assistant caregivers, verification of skills, tuberculosis clearance, presence of qualified staff when residents were present, and complete personnel records.

The manager failed to implement policies and procedures covering CPR training that includes a demonstration of the employee's ability to perform CPR for one of four personnel records reviewed.
The manager failed to ensure a caregiver provided documentation of completing a valid caregiver training program approved by the Department or NCIA Board for one of three caregivers reviewed.
The manager failed to ensure an assistant caregiver interacted with residents under the supervision of a manager or caregiver, allowing E5 to work alone unsupervised on multiple dates.
The manager failed to ensure an assistant caregiver's skills and knowledge were verified and documented before providing physical health services, as E5's skills checklist was undated.
R9-10-113 — The manager failed to ensure a caregiver and an assistant caregiver provided evidence of freedom from infectious tuberculosis on or before beginning work, with incomplete or untimely documentation for two personnel.
The manager failed to ensure at least one manager or caregiver was present at the assisted living home when a resident was present, as E5 worked alone on multiple dates.
The manager failed to ensure a personnel record included the individual's starting date of employment for one of four personnel records sampled, as E5's start date was not documented.
