Visit Reason
Annual Licensure Survey with multiple substantiated Entity Reported Incidents Investigations including IL00170052, IL00173360, IL00173938, IL00174530, and IL00177390.
Findings
The facility was found out of compliance with multiple regulations including residency requirements, disaster preparedness, incident reporting, personnel qualifications and training, employee orientation, health care worker background checks, Alzheimer's and dementia programs, resident rights, abuse prevention and reporting, and physical plant fire drill requirements.
Deficiencies (10)
A2000 (295.2000 Residency Requirements) - The establishment failed to ensure residents meet residency requirements for two residents requiring sliding scale insulin.
A2040 (295.2040 Disaster Preparedness) - The facility failed to ensure fire drills were conducted bimonthly, tornado drills on each shift in February, inclusion of residents in drills, and proper documentation of drill times and resident assistance.
A2050 (295.2050 Incident and Accident Reporting) - The establishment failed to submit incident reports to the department within 24 hours for two residents.
A3000 (295.3000 Personnel Requirements, Qualifications and Training) - The facility failed to ensure all nurses were correctly CPR certified, maintain tuberculosis clearance documentation, initial health evaluations, and comply with Health Care Worker Background Check Act for several employees.
