Visit Reason
Investigation of a complaint received on 2026-04-14 about medication errors, delayed treatment, and resident care.

Complaint Details
Resident A had a fall and delayed urinalysis collection: established. No documentation on staff training: not established. Medication called to the wrong pharmacy: established. Resident did not get out of bed and did not eat: not established.
Findings
Two rule violations were established related to delayed urinalysis collection and medication being called to the wrong pharmacy. Two other allegations regarding staff training and resident not eating were not substantiated.
