This report covers 13 inspections of Chesterbrook Residences, Inc. from March 15, 2021 to February 17, 2026, including routine, monitoring, renewal, and complaint investigations.

Three inspections were prompted by complaints regarding resident care, admissions, and buildings and grounds. Investigations included interviews, record reviews, and observations. Some complaints were not supported by evidence, while others resulted in violations.
