Residents exited unnoticed due to an unlocked door during a move-out, posing a risk to resident safety. This was a repeat deficiency from a prior complaint inspection.

R9-10-818 — The manager failed to ensure an evacuation drill for employees and residents was conducted at least once every six months, with no documentation of a drill after July 2024 available at the time of inspection.

R9-10-819 — The manager failed to ensure poisonous or toxic materials were stored in labeled containers in locked areas separate from food, dining, and medications, as multiple cabinets and rooms containing cleaning chemicals were found unlocked and accessible to residents.

The manager failed to ensure there was a means of exiting the facility for a resident without a key or special knowledge that controlled or alerted employees of the resident's egress, allowing a resident to exit through a door that was left ajar due to a malfunctioning auto-close feature.
