An on-site investigation of complaints AZ00216731, AZ00217260, and AZ00220242 was conducted on December 18, 2024 and the following deficiency was cited.
Findings
The inspection found one deficiency related to failure to provide appropriate first aid to a non-injured resident who had fallen and was unable to recover independently. The facility left the resident on the floor instead of assisting them off after the fall.

The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00205613 conducted on February 7-8, 2024.
Findings
This inspection found five deficiencies related to notification of resident death, incomplete medication storage plans, untimely service plan updates, missing medical determinations for non-ambulatory residents, and absence of posted pre-planned snacks on menus.
