Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility.

Findings
One violation was observed involving unsecured oxygen tanks in the cottage oxygen storage room. The violation was corrected at the time of inspection and the facility was approved.

Deficiencies (1)
IFC 5303.5.3 (2021) - Compressed gas containers, cylinders, and tanks must be secured to prevent falling by contact, vibration, or seismic activity. Two unsecured oxygen tanks were found in the cottage oxygen storage room.

The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.

Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements. Previously cited deficiencies were corrected.

WAC 388-78A-2230 Medication refusal. The facility failed to notify the physician when a resident refused medications, placing the resident at risk of health complications.
WAC 388-78A-2210 Medication services. The facility failed to implement a medication system by not following physicians’ orders to notify the physician when medications were not given, placing residents at risk of health complications.

The Office of the State Fire Marshal conducted a scheduled fire protection inspection at the facility to verify compliance with sprinkler system testing and maintenance requirements.

Findings
The inspection found that forward flow testing of the backflow preventers was required but scheduled and completed as of the inspection date. The facility was approved with no outstanding violations.

Deficiencies (1)
IFC 903.5 (2021) Sprinkler systems shall be tested and maintained in accordance with Section 901. Forward flow testing of the backflow preventers was required and scheduled for 7/1/25 with an extension until 7/2/25. Completed.

Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements. The prior deficiencies were corrected as documented.

WAC 388-78A-2140 Negotiated service agreement contents. The facility failed to ensure negotiated service agreements included a written plan with interventions to monitor and provide care for a resident with a suprapubic catheter, placing the resident at risk for improper catheter care and infection.
WAC 388-78A-2130 Service agreement planning. The facility failed to update negotiated service agreements after a change in resident condition for one sampled resident, placing the resident at risk of unmet care and services.
WAC 388-78A-2483 Tuberculosis One test. The facility failed to ensure staff received a one-step tuberculosis test, placing residents at risk of possible exposure to tuberculosis.
WAC 388-78A-2100 On-going assessments. The facility failed to ensure residents were assessed to safely smoke and use medical devices for three sampled residents, placing them at risk of injury due to lack of assessment.
