An on-site investigation of complaint AZ00213985 was conducted on August 13, 2024, and the following deficiencies were cited.
Findings
The inspection found four deficiencies related to documentation and service plans. Deficiencies included failure to provide required emergency responder documentation, incomplete service plan details, missing resident signature dates, and lack of weight documentation in service plans.

Service plan signatures — The manager failed to ensure a resident's written service plan was signed and dated by the resident or representative, as the date the resident signed was missing.
