Findings
The facility had 13 violations cited during the most recent inspection in January 2024, primarily related to management training and record keeping. Previous inspections in 2020 revealed multiple life safety code violations including fire safety and building maintenance issues, all of which were corrected.

The facility failed to have evidence showing that the manager completed the required training in the management of assisted living facilities.
The facility failed to have evidence showing that the manager completed the required 12 hours of annual continuing education.
The facility failed to search the NAR and EMR annually.
The facility failed to ensure the building and structure complied with the 2000 edition of the Life Safety Code, NFPA 101.
