The Department of Social and Health Services conducted a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.

Findings
The follow-up inspection found no deficiencies and confirmed that all previously cited deficiencies were corrected. The facility meets the Assisted Living Facility licensing requirements.

Deficiencies (4)
WAC 388-78A-2474 Training and home care aide certification requirements. The facility failed to ensure 2 of 6 sampled staff completed the required annual twelve hours of continuing education training, placing all residents at risk of unqualified care.
WAC 246-215-03500 Temperature and time control Frozen food (FDA Food Code 3-501.11). Stored frozen foods must be maintained frozen. Observation showed hot food served at 179 degrees inside the steam tray, which was corrected onsite by staff.
WAC 388-78A-2620 Pets. The assisted living facility must ensure animals have regular veterinary examinations and are certified free of diseases transmissible to humans. A feline examination dated 03/13/2023 was outdated and was updated during inspection.
WAC 388-78A-2950 Water supply. The facility must provide hot water between 105°F and 120°F at all times. Hot water temperatures in 3 of 9 sampled resident rooms were not maintained but were corrected onsite and verified within required range.

A complaint investigation was conducted due to a reported electrical fire in a light fixture at the facility.

Complaint Details
Complaint #137270 involved an electrical fire in a light fixture. The fire suppression system failed to activate, but the fire was extinguished by staff and the fire department responded. No injuries were reported.
Findings
A fire occurred in a resident unit bathroom and was extinguished by staff using a portable fire extinguisher. The fire suppression system did not activate. The area was evacuated, no injuries occurred, and the fire department responded.
