The complaint alleged that facility staff dropped Resident A causing a fractured femur. The allegation was substantiated based on interviews, record reviews, and staff statements confirming the incident and failure to use assistive devices.
Findings
The investigation confirmed that Resident A was dropped during a transfer from bed to wheelchair without the use of required assistive devices, resulting in an acute nondisplaced distal femoral fracture. Staff failed to follow Resident A's functional needs assessment which required use of a sliding board or assistive device.
