The facility failed to follow proper procedure when the memory care egress door alarmed, resulting in a resident eloping and posing an immediate safety risk. Staff received additional training and disciplinary action was taken against the responsible staff member.

No deficiencies were cited during the visit. The Licensing Program Analyst collected internal investigation documents and requested additional staff interviews to be conducted later.

The visit found that a resident sustained a hip fracture during a transfer maneuver without a fall. The resident's care plan was updated to include a hoyer lift to prevent future injury. No deficiencies were cited during the visit.

No issues were noted during the inspection. The facility was observed to be ready for licensing, with proper furniture, safety equipment, and environmental conditions.

The applicant and administrator successfully completed Component II of the licensing process via telephone call, confirming understanding of facility operation, admission policies, staffing, health conditions, emergency preparedness, complaints reporting, and pre-licensing readiness.
