Complaint Details
Several complaint surveys were conducted addressing allegations related to resident care, medication administration, staffing and supervision, building and grounds, and resident rights. Some complaints were substantiated with violations issued, while others were not supported by evidence. Complaint inspections included multiple visits and involved interviews, record reviews, and observations.
Findings
