The following deficiency was found during the on-site investigation of complaints 00145754 and 00142232 conducted on January 14, 2026.
Findings
The inspection found one deficiency related to failure to maintain a standardized emergency responder form for residents, which posed a risk if emergency responders lacked critical health information. A plan of correction was provided and accepted.

A.R.S. § 36-420.04.C — The manager failed to maintain a standardized emergency responder form for each resident including required information such as reasons for emergency responder contact, HIPAA release authorizations, advance directives, pharmacy and physician contact information. This deficiency posed a risk if emergency responders were not aware of critical health information.

The following deficiencies were found during the on-site compliance inspection and investigation of complaints 00105086, 00104982, and 00102489 conducted on August 19, 2025.
Findings
This inspection found six deficiencies related to personnel tuberculosis documentation, incomplete service plan documentation, improper medication storage, lack of refrigerator thermometer, incomplete incident report documentation, and unsecured poisonous materials. Plans of correction were provided for all deficiencies.

R9-10-817 — The manager failed to ensure medication was stored in a separate locked area used only for medication storage, posing a risk to residents not prescribed the accessible medication.
R9-10-818 — The manager failed to ensure a refrigerator used to store food contained a thermometer placed at the warmest part, risking potential food borne illnesses.
R9-10-819 — The manager failed to ensure that incident reports documented all required details after a resident's accident needing medical services, risking potential re-injury due to inadequate follow-up.
