On-site complaint investigation of complaints AZ00215638 and AZ00215591 combined with a compliance (annual) inspection at an Assisted Living Home, conducted 16 September 2024.

Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaints AZ00215638 and AZ00215591 conducted on September 16, 2024.
Findings
This inspection found 13 deficiencies related to personnel record compliance, policy reviews, abuse reporting, tuberculosis documentation, resident care documentation, resident dignity and privacy, facility modifications, and safety measures. Plans of correction were provided for all deficiencies.

Deficiencies (13)
The governing authority failed to ensure compliance with A.R.S. § 36-411.C by not documenting good faith efforts to contact previous employers for one of four personnel records reviewed.
The manager failed to ensure policies and procedures were reviewed at least once every three years and updated as needed, with no documentation available to confirm such review.
The manager failed to ensure compliance with abuse reporting requirements under A.R.S. § 46-454, lacking documentation of investigation and preventive actions for a suspected abuse incident.
R9-10-113 — The manager failed to ensure an employee provided documentation of freedom from infectious tuberculosis on or before the date services began, posing a potential TB exposure risk.
The manager failed to ensure caregivers documented services provided in residents' medical records for two residents, posing a health and safety risk.
The manager failed to ensure a resident was treated with dignity, respect, and consideration, as a resident's bedroom lacked a window and did not meet minimum size requirements.
The manager failed to ensure the means of exiting the facility for residents without keys or special knowledge controlled or alerted employees of egress; the alert device was switched off.
The manager failed to ensure a current toxicology reference guide was available for use by personnel members; the facility had an outdated 2nd edition instead of the current 4th edition.
The manager failed to ensure a current therapeutic diet manual was available for use by employees, risking inadequate dietary information for residents.
The manager failed to ensure hot water temperatures were maintained between 95º F and 120º F in resident areas; observed temperature was 142º F, posing a health and safety risk.
