Complaint Details
The complaint investigation (Compliance Determination #58609) was triggered by an allegation that staff did not report a fall with injuries to management staff. The investigation found that staff failed to implement the negotiated service agreement and failed to report the fall timely, resulting in citations.
Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements. Previously cited deficiencies under WAC 388-78A-2160 and WAC 388-78A-2640-1-a were corrected.

WAC 388-78A-2640 Reporting significant change in a resident's condition. The assisted living facility failed to ensure timely reporting of a fall with injuries to management staff, the resident's representative, and physician for one sampled resident, resulting in delayed care and pain relief.
