Findings
The follow-up inspection on 06/25/2024 found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements. The prior deficiencies were corrected as documented.

The assisted living facility failed to develop and document in the resident's record the agreed upon plan to address and support each resident's assessed capabilities, needs, and preferences, including frequencies of care tasks for 7 of 8 residents.
WAC 388-78A-2480 Tuberculosis Testing Required. The assisted living facility failed to ensure that staff had the initial tuberculosis skin test completed within three days of hire for 1 of 4 staff, placing residents at risk of exposure to communicable disease.
