Visit Reason
On-site compliance (annual) inspection at Firebird Assisted Living #2 conducted on August 19, 2024.
Findings
This inspection found five deficiencies related to documentation, facility safety, and equipment maintenance. The facility failed to meet requirements for resident documentation, means of egress, fire extinguisher mounting and servicing, and swimming pool gate security.
Deficiencies (5)
The manager failed to ensure residents submitted documentation dated within 90 calendar days before acceptance regarding their need for continuous medical services, nursing services, or restraints. This posed a risk if the facility was unable to meet residents' needs.
The manager failed to ensure there was a means of exiting the facility for residents without keys or special knowledge that provided access to an outside area at least 30 feet away and alerted employees of egress. This posed a risk if the facility was unaware of residents' whereabouts.
The manager failed to ensure a fire extinguisher was mounted in the assisted living home. The extinguisher was observed on a countertop but not mounted.
The manager failed to ensure a rechargeable fire extinguisher was serviced at least once every 12 months. This posed a health and safety risk if the extinguisher was needed and did not work properly.
The manager failed to ensure the swimming pool was enclosed by a wall or fence with a self-closing, self-latching gate that was locked when the pool was not in use. The pool gate was observed unsecured with the key in the keyhole.
