Visit Reason
The purpose of this visit was to conduct the compliance inspection with an onsite visit made to the facility on 2/7/24.

Findings
The facility failed to meet several workforce qualification and training requirements, including ensuring staff received at least 16 hours of training per year, tuberculosis screening and physical examinations prior to employment, and criminal fingerprint background checks. Additionally, the facility failed to maintain a medication assistance record (MAR) for one sampled resident.

Facility failed to ensure staff involved with personal services had at least sixteen (16) hours of training per year for 4 of 4 sampled staff.
Facility failed to ensure each employee received a tuberculosis screening and physical examination within twelve months prior to employment for 1 of 4 staff (Staff C).

The purpose of this visit was to conduct a compliance inspection.

Findings
The facility failed to maintain a complete Medication Assistance Record (MAR) for 2 of 3 sampled residents, omitting several medications including hospice drugs. Additionally, the facility failed to properly dispose of unused and expired medications, as evidenced by a medication prescribed to a former resident remaining in the medication cart.

Deficiencies (2)
The MAR did not include the name, strength, summary of severe side effects, and a chart for staff to record medication administration for 2 of 3 sampled residents.
Failed to properly dispose of unused and expired medications; Bisacodyl 10 mg prescribed to a former resident was found in the medication cart.
