On-site compliance (annual) inspection of an Assisted Living Home conducted on April 22, 2024.
Findings
This inspection found six deficiencies related to documentation, safety, and medication storage practices. The facility failed to maintain required signatures on residency agreements, ensure functional exit alarms, provide a current drug reference guide, and properly secure medications and hazardous materials.

The manager failed to ensure the residency agreement included the manager's signature and date signed for one of two residents reviewed.
The manager failed to ensure a means of exiting the facility for residents without keys or special knowledge included a working device to alert employees of egress; the alarm device was switched off and nonfunctional.
The manager failed to ensure a current drug reference guide was available for use by personnel; the facility used a 2020 edition instead of the 2024 edition.
The manager failed to ensure medication was stored in a separate locked area; medication cups with multiple medications were found in an unlocked kitchen drawer.
The manager failed to ensure poisonous or toxic materials were stored in labeled containers in a locked area inaccessible to residents; a can of cleaner was found in an unlocked garage.
The manager failed to ensure combustible or flammable liquids and hazardous materials were stored in a locked area inaccessible to residents; four cans of butane gas were found in an unlocked garage.
