Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on August 29, 2024.

Findings

Medication assistance/Emergency call response: Documentation of hourly safety checks for 27 Special Care Residence residents had multiple instances of missing hourly safety checks during the hours of 7:00 p.m. and 7:00 a.m. (repeat finding).
Quality assurance program: Progress note documentation indicated that care staff administered cream and powder medications outside the scope of self-administered medication management.
Incident reporting: Multiple incidents meeting reporting criteria were not reported as required.
Resident progress notes: Inconsistent documentation of significant occurrences was found, including missed or delayed scheduled care tasks and medications in ten resident records.
Resident records: Correspondence logs did not consistently document all significant information necessary to maintain continuity of care, including missed or delayed scheduled care tasks.
Staffing/Memory care unit: Documentation confirming timely staffing level assessments was missing, and staffing was insufficient to address scheduled and unscheduled needs (repeat finding).
Staff training: Documentation of Introductory Visits for residence and agency staff was inconsistent and incomplete (repeat finding).
