Visit Reason
The Minnesota Department of Health conducted a survey on April 16, 2025, to evaluate and assess compliance with state licensing statutes for Gleseners Assisted Living.

Findings
The licensee was found in substantial compliance but had several violations including food service not fully compliant with Minnesota Food Code, lack of a current tuberculosis prevention program, incomplete emergency preparedness plan, failure to provide required notice of designated representative rights, medication administration error, incomplete individualized treatment plan, and missing electronic monitoring signage.
