Allegations included lack of assistance with bathing, missing staff criminal clearances and health screenings, inadequate special diets, unsafe food handling, insufficient administrator presence, poor food quality, unclean linens, facility dirtiness, lack of activities, and misuse of facility resources. All were found unsubstantiated or unfounded.
