Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on February 27, 2025.

Findings
The state cited 8 findings, including 5 repeat findings, which is above the state median of 6. The action taken was a plan of correction required.

Medication assistance and Emergency call response: Documentation of actions regarding whether the Resident took or refused medication was missing for two Residents, and documentation of monitoring e-call response times over the eight minute limit was missing for January 2025 (repeat finding).
Memory care unit: Several physical hazards were identified including unsecured cleaning supplies, unsecured microwaves without safeguards, unsecured laundry and mechanical closet doors, and an unsecured tool shed door.
Resident screening and assessment and Resident care plans: Documentation was missing for service plan reassessments, signatures, service plan reviews, individualized enrichment activities, 24-hour awake onsite staff, and assessments by a physical/occupational therapist for residents using bed rails or U-bars (repeat finding).
Quality assurance program: Missing or incomplete documentation of audit dates, auditors, and identifying records for 2023 and 2024; medication storage units contained unidentified loose pills, expired medications, discontinued medications, improperly labeled cassettes, and [redacted] without documentation (repeat finding).
Incident reporting: The Residence filed 81 incident reports greater than 24 hours after the occurrence of the incident or accident (repeat finding).
Recordkeeping: Three personnel records were missing documentation of a signed job description.
Staff training: Documentation of general orientation was incomplete for seven staff; introductory visits were inconsistently documented; biannual SAMM and skills evaluations were missing for some staff; required disclosure forms were not signed by residents or representatives (repeat finding).
