Visit Reason
On-site annual compliance inspection of an Assisted Living Home conducted on February 2, 2024.
Findings
The inspection found six deficiencies related to staff training on fall prevention, vaccination documentation, resident retention criteria, evacuation orientation, fire extinguisher maintenance, and tuberculosis infection control. Plans of correction were provided for all deficiencies.

36-420.01 — The manager failed to ensure the facility administered continued competency training for all staff on fall prevention and fall recovery, posing a risk to resident safety.
The manager failed to ensure a resident's medical record contained current documentation of notification regarding availability of influenza and pneumonia vaccinations, posing a potential illness risk.
R9-10-814 — The manager failed to ensure the facility did not retain a resident confined to a bed or chair without obtaining a written determination every six months that the resident's needs could be met and were within the facility's scope of services.
The manager failed to ensure a resident received orientation to the facility exits and evacuation route within 24 hours of acceptance, posing a health and safety risk in emergencies.
The manager failed to ensure a rechargeable fire extinguisher was serviced at least once every 12 months, posing a health and safety risk to residents.
