An on-site compliance inspection and investigation of complaints 00104979 and 00105010 was conducted on January 7, 2026, and the following deficiencies were cited:
Findings
Two deficiencies were cited involving tuberculosis testing documentation for personnel and incomplete residency agreements regarding awake nighttime staff. Both deficiencies were repeats from a prior inspection.
Deficiencies (2)
R9-10-806 — The manager failed to ensure employees provided documentation of freedom from infectious tuberculosis as required, posing a potential TB exposure risk to residents. One of three personnel sampled lacked the required second TB skin test.
R9-10-807 — The manager failed to ensure residency agreements included whether a manager or caregiver was awake during nighttime hours for two residents. The agreements lacked clear verbiage on this requirement.
