Visit Reason
On-site compliance (annual) inspection of an Assisted Living Home conducted on February 19, 2026.

Findings
The inspection found four deficiencies related to staff training on fall prevention, tuberculosis infection control activities, tuberculosis screening documentation, and securing oxygen containers. Plans of correction were provided for all deficiencies.

R9-10-113 — The chief administrative officer failed to provide annual tuberculosis training and education and did not conduct an annual tuberculosis risk assessment for two personnel, posing a potential illness risk.
R9-10-806 — The manager failed to ensure that two personnel provided evidence of freedom from infectious tuberculosis as required, including missing risk assessment and incomplete two-step TB skin test documentation.
R9-10-820 — The manager failed to ensure oxygen containers were secured in an upright position, as three tanks were found unsecured, posing a potential explosion or gas leak risk.
