The following deficiencies were found during the on-site abbreviated follow-up inspection and investigation of complaint 00124486, conducted on November 4, 2025.
Findings
Two deficiencies were found related to failure to ensure services were provided and documented according to the resident’s service plan and illegible entries in medical records. The facility provided plans of correction for both deficiencies.
Deficiencies (2)
R9-10-808 — The manager failed to ensure a caregiver assisted with activities of daily living according to the resident’s service plan and failed to document the application of an abdominal binder as ordered in the resident's medical record.
R9-10-811 — The manager failed to ensure entries in residents’ medical records were legible, as service plans contained numerous illegible handwritten notes.
