Visit Reason
The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.
Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets the Assisted Living Facility licensing requirements. Previously cited deficiencies were corrected as listed in the letter.
Deficiencies (5)
WAC 388-78A-2665 Resident rights Notice Policy on accepting medicaid as a payment source. The facility failed to ensure 7 sampled residents were verbally informed and given a copy of the Medicaid payment policy with signed acknowledgement.
WAC 388-78A-2484 Tuberculosis Two step skin testing. The facility failed to ensure 1 of 6 sampled staff completed an initial TB skin test within three days of hire, placing 44 residents at risk of exposure.
WAC 388-78A-2320 Intermittent nursing services systems. The facility failed to ensure 2 of 2 sampled residents who used oxygen received oxygen as ordered, placing them at risk for medical errors and health decline.
WAC 388-78A-2140 Negotiated service agreement contents. The facility failed to develop and document on the Service Plan the use of blood thinners or insulin for 4 of 4 sampled residents and the discontinuation of catheter care for 1 sampled resident, placing residents at risk for unmet care needs.
WAC 388-78A-2100 Ongoing assessments. The facility failed to assess safety considerations for a medical device and smoking tobacco products for 6 of 7 residents, placing them at risk of injury or compromised health.

Complaint Investigation

Visit Reason
The inspection was conducted in response to a complaint (#195230) alleging smoking violations at Hillside Assisted Living.
Complaint Details
Complaint #195230 alleged smoking violations. The investigation included observations and discussions with facility staff. No IFC violations were found and the complaint was effectively unsubstantiated.
Findings
The investigation found no fire code violations and confirmed that the facility had designated a smoking area where residents were observed smoking. No injuries were reported.

Life Safety

Visit Reason
On 9/9/25 an announced Fire and Life Safety Code Inspection was conducted at Stafford Suites Kent, now known as Hillside Assisted Living, by the State Fire Marshal's Office to determine compliance with all applicable codes.
Findings
The inspection found multiple fire and life safety code violations including improper use of extension cords, penetrations in fire-resistance-rated construction, emergency lighting failure, and blocked exit access. All violations were noted as corrected on site except the overall approval status remained Disapproved.
Deficiencies (4)
IFC 603.6 (2021) Extension cords shall not be used as permanent wiring and must be listed and labeled per UL 817. An extension cord was used as permanent wiring in room 104.
IFC 703.1 (2021) Materials and firestop systems must maintain fire-resistance-rated construction and prevent passage of smoke. Penetrations were found in fire-resistance-rated construction in the staff break room above the door and basement by holiday storage.
IFC 1032.10 (2021) Emergency lighting must be maintained and tested per code. Emergency light across from room 213 failed to operate on backup power during testing.
IFC 1032.2 (2021) Required exit accesses and exits must be free from obstructions to allow full use in emergencies. A table and chair were blocking means of egress in the dining area.
