Visit Reason
Annual Licensure Survey conducted to assess compliance with physician assessment and service plan requirements for residents.
Findings
The facility failed to ensure that physician assessments were completed by a physician for two residents, as assessments were completed by an Advanced Practice Nurse Certified Nurse Practitioner. Additionally, the facility failed to include negotiated risk agreements in the service plans for the same two residents.
