Plan of correction responding to a survey citing violations of Sections 295.4000 and 295.4010.
Findings
The facility committed to ensure that a Physician’s Assessment will be completed, dated, and signed by a physician before every new admission, annually, and upon significant change in condition. The Resident’s Service Plan has been modified to include the negotiated risk agreement as required.
