Visit Reason
The inspection was conducted as a license renewal survey for the Level III Residential Care Facility named Lark.

The facility was found to be non-compliant with multiple regulatory requirements including failure to provide signed admission contracts, informational packets to residents, current written consent to release information, notification of resident rights, timely assessments and service plans, documentation of progress notes, completeness of resident summary sheets, and proof of legal guardianship or representation.
