Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility to assess compliance with fire protection and safety codes.
Findings
The inspection identified several violations related to fire alarm system use, fire wall inventory, carbon monoxide detector maintenance, and fire/smoke damper testing. All noted deficiencies were corrected or addressed, and the facility was approved.
Deficiencies (4)
IFC 405.8, 2021 - Facility did not use the fire alarm system to initiate fire drills for the July and October 2025 swing shift drills. The alarm must be activated for all drills except during the period from 9 p.m. to 6 a.m. when a simulated alarm may be used.
IFC 701.6 2018 WAC 51-54A - Facility is unable to provide annual fire wall inventory (floor plans), but has completed quarterly inspections of the fire walls.
IFC 915.6 2021 WAC - Facility was unable to provide documentation for monthly carbon monoxide detector maintenance for October, November, December 2025 and January through May 2026. Carbon monoxide alarms and connected appliances must be inspected and tested monthly per manufacturer's instructions.
NFPA 80 - Failed fire/smoke damper items identified during 5/15/25 inspection were repaired on 6/26/25. Each damper must be tested and inspected one year after installation and periodically thereafter.

Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility on 06/26/2025.
Findings
The inspection found some deficiencies including lack of documentation for the 4-year fire and smoke damper inspection, but these were scheduled for follow-up. Other issues such as systems out of service and testing were corrected or completed. The overall approval status is Approved.
Deficiencies (4)
IFC 901.7 (2021) Where a required fire protection system is out of service, the fire department and fire code official shall be notified immediately and fire watch provided until service is restored. Corrected.
IFC 903.5 (2021) Sprinkler systems shall be tested and maintained in accordance with Section 901. Forward flow testing of the backflow preventers shall be required. Completed.
NFPA 80 Fire/Smoke Dampers Inspection and Testing Facility is unable to provide documentation for the 4 year fire and smoke damper inspection. Scheduled for 5/14/25 with report to be provided on follow-up.
WAC 212-12-025 Any facility licensed by the department of health or social and health services shall report within 24 hours any accidental or unintentional fire or unusual incident threatening the facility. No violation noted.
