Annual Licensure Survey with violations cited in sections 295.2040 a) c) d), 295.3030 a) e) 1) 2), and 295.3040 - 955.165 a) d) 1).
Findings
The establishment failed to conduct required tornado and fire drills on schedule, did not ensure initial health evaluations and tuberculosis testing for all direct care and food service employees, and delayed fingerprint-based criminal history checks for some employees.

A2040 (295.2040 Disaster Preparedness) - The establishment failed to conduct the tornado drill in February 2025 and did not conduct fire drills on a bi-monthly basis, missing drills between April and July 2025.
