Findings
The inspection identified deficiencies related to sprinkler system maintenance and fire alarm panel security. The facility was disapproved due to these unresolved fire safety issues.
Deficiencies (2)
IFC 903.5 (2021) - Sprinkler systems shall be tested and maintained in accordance with Section 901. Kitchen area has multiple sprinkler heads loaded with debris.
IFC 907.8.2 (2021) - Maintenance and testing schedules for fire alarm and detection systems must be maintained. Electrical panel did not have lock on the fire alarm electrical breaker.

WAC 388-78A-2480 Tuberculosis Testing Required. The facility failed to keep a record of tuberculosis testing for 1 of 6 sampled staff, placing residents at risk of TB infection.
WAC 388-78A-2610 Infection control. The facility failed to provide a full Respiratory Protection Program including N95 mask fit testing for 6 of 6 sampled staff, increasing risk of COVID-19 infection.
WAC 388-78A-2040 Other requirements. The facility failed to ensure the designated smoking area was 25 feet away from the building, exposing residents and visitors to secondhand smoke.
WAC 388-78A-2464 Background checks Process Background authorization form. The facility failed to ensure 1 of 6 sampled staff received their Name and Date of Birth background check timely, placing residents at risk of being cared for by disqualified staff.
WAC 388-78A-2474 Training and home care aide certification requirements. The facility failed to provide documentation of CPR and first aid training for 1 of 6 sampled staff, placing residents at risk for unqualified emergency care.
WAC 388-78A-24681 Background checks Employment Provisional hire Pending results of national fingerprint background check. The facility failed to ensure 1 of 6 sampled staff received their final fingerprint background check within 120 days of hire, placing residents at risk of being cared for by staff with potentially disqualifying criminal history.
