The complaint investigation was substantiated. The complaint alleged that involuntary discharge requirements were not followed. The provider failed to provide the required 30-day written notice and did not ensure a suitable living arrangement was available before discharge.

The investigation found that the provider did not ensure Resident 1 or their power of attorney received a 30-day written advance notice before initiating an involuntary discharge, including appeal rights and assistance for relocation.
